Who can apply
- The applicant must be pregnant.
- Applicants in the listed Maryland ZIP codes, age 18 or older, no more than 27 weeks pregnant and eligible for Medicaid.

- A MyChart screenshot, after-visit summary, WIC referral form, or urine or blood test result.
- Must show patient name, provider name, visit date, and due date or gestational age.
- The visit must be within the last 3 months.
- Sonograms, photos of pregnancy tests, and notes from providers are not accepted.

Please check to make sure you meet the requirements below before you apply. If your application is selected to move forward, The Bridge Project will ask you to submit documents verifying you meet these requirements. You do not need a Social Security Number or ITIN to apply.

- If randomly selected, you will receive time-sensitive email instructions for the verification stage.
- The application says you will receive an update within two months.

Before you send it
- Use the pregnancy proof types the application accepts. Sonograms, photos of pregnancy tests, and notes from providers are not accepted.
What we could not check
- The cohort page says it is enrolling, but the main application page says unlisted locations are closed. Confirm before applying.
- Published application closing dates and exact payment amounts or schedules were not found on the captured pages.
