Check if you qualify in a few minutes, then apply on The Bridge Project's own form. The Bridge Project decides who is chosen.
Who can apply
- The applicant must be pregnant.
- Milwaukee residents age 18 or older who are no more than 23 weeks pregnant and meet the income rule.
Documents to gather
Proof of pregnancy
- A MyChart screenshot, after-visit summary, WIC referral form, or urine or blood test result.
- Must show your full name, provider name, visit date within the past 3 months, and due date or how far along you are.
- Upload a JPG, PNG, WEBP, or PDF file, up to 20 MB.

- If the application moves forward, the Bridge Project will email the next steps.

Before you send it
- Use the applicant's details if you are helping someone else.
- For Milwaukee, count income for you and your spouse. Do not include income from other relatives who live with you.
- Check whether the pregnancy document uploaded. If only the document failed, use the retry option.
